- Driving Safety Course Request affidavit (JP5: sworn before a notary or the clerk; notarized if mailed) with plea of guilty or no contest and jury waiver
- Copy of a valid Texas driver license or permit (or active military ID)
- Proof of financial responsibility (liability insurance on the vehicle operated)
- Type 3A DPS driving record (JP5 wants it attached to the request; JP6 wants a DPS record by the completion due date)
- Payment of $144 (cash, cashier's check or money order; no personal checks by mail; JP6 takes no partial payments)
Court costs $134 plus a $10 reimbursement fee ($10 driving safety course fee plus $134 court costs, total $144, stated by JP5 and JP6; JP5 wants it paid with the request; JP6 emailed requests get online payment instructions after the plea is received; the other precincts say contact the court before paying).
You have 90 days to finish the course and turn in the court copy of your certificate, your Type 3A driving record and the notarized affidavit. The court's page does not say how to file them, so ask the clerk when you are approved. The affidavit must be notarized, or signed in front of the clerk where the court allows it.
Only JP5 and JP6 publish a DSC procedure; JP1, 2, 3, 4, 7 and 8 say to contact the court before paying online if you want defensive driving. County-wide rule: request in person or by mail with a sworn affidavit, notarized if mailed, and pay $144. JP6 also accepts the written request and plea by email at jp6clerk@tarrantcountytx.gov with a copy of insurance. JP5 counts the 90 days from the request. Neither precinct states how the certificate is filed (JP5: sign the copy and file it with the court immediately), so confirm with your precinct. Missing the completion deadline leads to a show cause hearing and, if you miss that, a conviction, a $200 fine and a warrant. No online DSC request portal
