The court's page does not say how to file them, so ask the clerk when you are approved. The affidavit must be notarized, or signed in front of the clerk where the court allows it.

Good to know

The 90 days run from the date you sign the court's affidavit. In person the affidavit is sworn before the clerk; by mail it must be notarized. The online request is a web form with a sworn statement, a plea of guilty or no contest, and attachments proving license, insurance and eligibility; the precinct comes from the citation. The page says to file the signed court copy of the certificate but not by which method, and it names a DPS driving record without saying Type 3A, so confirm both with your precinct.
